About the Provider
As a Family Medicine Physician, Dr. David Tomazic specializes in the diagnosis and treatment of acute and chronic conditions in teens and adults and acts as a patient’s primary care provider. He provides screenings, vaccinations, orders diagnostic testing and coordinates care with specialists when necessary.
Dr. Tomazic joined the staff of Wayne Memorial Community Health Centers in 2016 with a history of more than 30 years in a solo private practice serving the greater Forest City area. In addition to outpatient primary care, Dr. Tomazic serves as the Medical Director of the Forest City Nursing and Rehabilitation Center.
Dr. Tomazic also held the position of Medical Director of the State Correctional Institute in Waymart for 13 years.
