Our Services
Wayne Memorial Community Health Centers offer primary care services across ten sites. We have offices located in Honesdale, Hamlin, Carbondale, Forest City, Waymart, Tafton, Lords Valley, Lake Como, and Lake Ariel. Our staff of physicians, advanced practice providers and support professionals work as a team to provide patients with the most coordinated and responsive care possible. Services include routine exams and immunizations, treatment and diagnosis of acute conditions, management of chronic diseases and referral to specialists as needed.
