- Behavioral Health Screening
- Real time assessment
- Preventative care through a team approach
- Brief or expanded therapy on a flexible schedule
- Psychiatric assessment and short-term stabilization
How can I get started?
To access Integration of Care Services, individuals must be an active patient with Wayne Memorial Community Health Centers. Talk to your provider and they can make a referral.
